Here are the steps:

1. Go to gplus.name

2. Enter your Google+ ID in the first space. Your ID will look like this: https://plus.google.com/109298887003397397320/posts/……….
My Google ID is: 109298887003397397320

3. Put in your desired name and click submit.

4. You now have your unique Gplus.name.
